Questions 1-8 score 1 point for each correct answer:

3. Nicholas Fury first appeared in Marvel Comics in Sgt. Fury & his Howling Commandos #1.

However, as predicted by his doctor at the time, years later the old wound eventually caused Fury to lose sight in the left eye permanently. This was the trick Marvel used when they told the story of the origin of the eye patch in WWII but had to explain how in the early sixties Nick Fury showed up here and there in cameos as a CIA operative without the eye patch. Somewhere between appearing as a government liason to the Fantastic Four and becoming director of SHIELD, he permanently lost sight in that eye.

19. When Marvel and DC united to create the "Amalgam" line of books, they merged Nick Fury with Bruce Wayne to create "Bruce Wayne, Agent of SHIELD." On a side note, Bat-Man (Bruce Wayne's alter-ego_ was merged with Wolverine to create Dark-Claw. And Sgt. Rock showed up too in this issue as a compaion to Sgt. Fury.
20. Marvin Flumm, a.k.a. Mentallo, was the first rogue SHIELD agent to become a stand-alone super-villain. See Strange Tales #141. While there may have been Hydra infiltrators inside SHIELD prior to this, none of those namesless hoods became a super-villain on their own within the Marvel Universe.

Again, seeing as how the "Fallen Angel" is yet to be identified, he could be the correct answer to this question as identified by the Fury one-shot issue which followed the Nick Fury vs. SHIELD limited series. According to that one-shot. Rick Stoner was SHIELD's first director, who Fury was passed over for the first time around. When Stoner was killed by Hydra shortly into his directorship, Nick Fury was reconsidered and subsequently offered the position.
25. Scorpio was Jacob "Jake" Fury, Nick's younger brother. Twisted by his hatred for Nick, he helped create the organization called the Zodiac and used an energy weapon called the Cosmic Key. His nephew is Scorpio, Nick's long lost son who spent most of his life mistakenly believing he was Jacob Fury's son (see the Scorpio Connection). Again, to get the 3 points you must have gotten all 4 parts of this question correct - no partial points!
